Developmental Disabilities and Community Options

Program Description: The Developmental Disabilities Unit provides intake, assessment, case planning, ongoing coordination, and case coordination services to individuals with developmental disabilities.

In addition, housing, vocational training, transportation, respite care services, and Family Support Program Services, along with other ancillary services are purchased and monitored through this unit. Enrollment in federal, state, or other funding programs may be recommended to help pay for care and other needed services.

  • Cooperating with school systems in IEP transition planning.
  • Provide student and family assessment to encourage long term planning.
  • Promote awareness of adult disability related services.
  • Gather information and feedback from current service providers to assist in planning for adult living.
  • Individual and family counseling.

  • Alternate care for children.
  • Court intake and studies.
  • Supportive home care.
  • Child day care.
  • Specialized transportation.
  • Home and financial management.
  • Juvenile probation and supervision.
  • Child and elderly abuse services.
  • Crisis intervention
  • Information and referral and health-related services.
  • Income assistance programs including-food stamps, energy assistance, medical assistance.
  • Outpatient clinic dealing with issues of mental health, developmental disabilities and alcohol and other drug abuse.
  • Birth to three program.
  • Community Options Program-assists persons in maintaining community living.
  • Community Support Program-assists long term mentally ill to maintain community living and provides home-based therapy for those with developmental delays

  • Emergency crisis intervention and advocacy.

 

Call to make a referral or appointment.

  • Services free to SSI recipients, other persons and with income limits.
  • Fees are based on ability to pay, third party payments can be applied for.
