Sheboygan County Health and Human Services;

Long-term Support - Developmental Disabilities

 

 

Services are Board operated and contracted and include such services as case management, personal/family counseling, vocational rehabilitation, Birth to Three Services, residential options, family support, respite care and protective services.

- Case Management and Couseling
Assessment and evaluations of individuals to determine what services are needed.

- Coordinate those services and provide follow-along to clients.
Respite Care
Provide relief to 24-hour caregivers of children and adults with developmental disabilities.

- Residential Services
Services to individuals living in situations other than a natural home, i.e., adult family homes, group homes, and sheltered apartments.

- Vocational and Day Services
Services are contracted through community agencies and include supported employment, sheltered employment and day services.

- Birth to Three Program
Early Intervention services provided to children with disabilities ages 0-3 years.
Community Integration Program (1A and 1B)
Funding program designed to divert individuals with developmental disabilities from state centers and ICF-MR Units to community living arrangements.

- Family Support Program
Funding program designed to assist families in caring for their children with disabilities in the natural home setting.

- Protective Services
Facilitate and evaluate for guardianship and protective services for developmentally disabled individuals.
